Data subjects, defined as "data subjects" under Law No. 6698 on the Protection of Personal Data (the "Law"), have been granted certain rights regarding the processing of their personal data under Article 11 of the Law.
Under Article 13(1) of the Law, applications made to our Company, as data controller, regarding these rights must be submitted to us in writing or through other methods determined by the Personal Data Protection Board (the "Board").
In this context, applications made to our Company "in writing" must be submitted to us by printing this form and:
- By the applicant's personal application,
- Through a notary,

C. Information on the data subject's choice of right to exercise
- I want to learn whether your Company processes personal data about me.
- If your Company processes personal data about me, I request information about these processing activities.
- If your Company processes personal data about me, I want to learn the purpose of processing and whether it is used in accordance with that purpose.
- If my personal data is transferred to third parties domestically or abroad, I want to know these third parties.
- I believe my personal data has been processed incompletely or incorrectly and I request that it be corrected.
- Even though my personal data has been processed in accordance with the Law and other relevant legal provisions, I request that my personal data be deleted.
- I request that the correction of my personal data, which I believe was processed incompletely or incorrectly, also be made with third parties to whom it was transferred.

- I believe that personal data processed by your Company has been analyzed exclusively through automated systems and that this analysis has resulted in an outcome to my detriment. I object to this outcome.

This application form has been prepared so that, by identifying your relationship with our Company and fully determining any personal data processed by our Company, your application can be answered accurately and within the legal timeframe. In order to eliminate legal risks that may arise from unlawful and unauthorized data sharing, and in particular to ensure the security of your personal data, our Company reserves the right to request additional documents and information (such as a copy of an ID card or driver's license) for identity and authorization verification.
If the information you provide as part of your request is inaccurate or outdated, or if an unauthorized application is made, our Company accepts no liability for claims arising from such inaccurate information or unauthorized applications.
Applications submitted to us will, in accordance with Article 13(2) of the KVK Law, be answered as soon as possible from the date the request reaches us through one of the methods described above, depending on the nature of the request. Our responses will be delivered to the applicant in writing or electronically in accordance with Article 13 of the KVK Law. Whether we respond in writing or electronically will depend on your choice. If you request a written response and the response exceeds 10 pages, a fee may apply for pages beyond the 10th, and if an electronic response is given, the cost of the recording medium may be charged to you.
Applicant (Data Subject)
Pursuant to the Communiqué on Data Controller Application Procedures, all fields in Section "A" identifying the applicant, and all other fields of the form, must be completed in full and the request clearly stated. Otherwise, the application will not be considered a valid application.
